FINAL RULE ] On
July 31, HHS issued a rule (CMS-0043-F) finalizing October 1, 2015, as
the new compliance date for health care providers and health plans to
transition to ICD-10. ICD-10 represents a significant code set change that

Completeness of documentation
While there are
many different ways to document all the required information, and tools like tailored
EHR templates and computer assisted coding (CAC) programs can help construct
a complete and detailed record, physicians must be trained to be as thorough
as humanly possible when they’re composing notes. Since ICD-10 takes
into account more variables than the current code set, physicians are better
off writing down more information than less.
“ICD-10 will not
affect the way you provide patient care,” CMS reminds physicians. “It will
just be important to make your documentation as detailed as possible since
ICD-10 gives more specific choices for coding diagnoses. This information is
likely already being shared by the patient during your visit—it’s just a
matter of recording it for your coding staff. Good documentation will also
help reduce the need to follow-up on submitted claims—saving you time and
money.”
