The facts of ICD-10 :

The role of the clinician is to document as accurately as possible the nature of the patient conditions and services done to maintain or improve those conditions.

 The role of the coding professional is to assure that coding is consistent with the documentation
 The role of the business manager is to assure that all billing is accurately coded and supported by the documented fact.

As you can see, it requires all parties to have an understanding of ICD-10.  For providers, coders, and administrators to solely rely on the EHRs to capture coding, the mapping of ICD-9 to ICD-10 is a big mistake. The time is now that CMS has given instructions NOT to deny claims, reduce payments, or call for audits to get a full understanding of ICD-10. Failure to use ICD-10 correctly will impact you financially.
